Brief Title:    Agreement with CommuniCare Health Centers for Project Room Key Services

Approve agreement with CommuniCare Health Centers, Inc. in the amount of $14,227.86 for the period of September 1, 2021, through September 30, 2021, for the provision of project room key services. (No general fund impact) (Larsen)
Recommended Action
  1. Approve agreement with CommuniCare Health Centers, Inc. in the amount of $14,227.86 for the period of September 1, 2021, through September 30, 2021, for the provision of project room key services; and
  2. Delegate authority to the Director of the Yolo County Health and Human Services Agency or her/his designee to: 1) exercise optional monthly extensions up to $14,227.86 per month and 2) issue related optional notices. Please see Section III. of Att. A. Agreement.

Reason for Recommended Action/Background
The California Department of Social Services (“CDSS”) released a Project Room Key (“PRK”) allocation of funding to continue to provide Project Room Key and Rehousing strategy for non-congregate shelter and rehousing services.
CommuniCare will provide primary support/coverage for individuals identified as eligible in the City of Davis to access a motel room for their quarantine/isolation period. CommuniCare will take the lead as the Primary Motel Homeless Services point of contact. CommuniCare will provide adequate staff available to respond Monday through Friday 8:00 a.m. to 5:00 p.m. as well as on-call coverage from 8:00 a.m. to 8:00 p.m. on weekends and holidays. CommuniCare will respond to requests for immediate need such as securing motel rooms, meals, transportation, medication, and other supplies as determined to be required for client safety while determining eligibility for access to services
For the ease of ongoing administration of this agreement, staff also requests the approval of the HHSA Director to execute monthly optional extension notices.

No general funds are required by this action. These services will be funded by the California Department of Social Services (CDSS) Project Room Key (PRK). The total amount of this action is $14,227.86 for the period of September 1, 2021, through September 30, 2021. The amount of $14,227.86 is included in the HHSA recommended budget for fiscal year 2021-22.

In no event shall the term of the Agreement extend beyond December 31, 2021, nor shall the maximum payment obligation exceed the amount of FIFTY-SIX THOUSAND NINE HUNDRED ELEVEN DOLLARS AND FORTY-FOUR CENTS ($56,911.44), unless otherwise agreed to in writing by the parties and in conformity with the then-current Yolo County Procurement Policy approved by the Yolo County Board of Supervisors.
