Headlinerz Barbershop and Salon, LLC. located at 2259 E.42nd Street, Lorain OH 44055 to receive a Forgivable Loan in the amount of $7,383.75. This is an amendment to the original approved Board of Control date of February 24, 2021, due to an account and amount change. Account #:1010.G800.4950.1304

b.  
A request from the Engineering Department for a purchase order in the amount of $702,997.93 to pay Eschtruth Investment Co, LLC., via wire transfer, to acquire approximately 18.6493 acres of property located on Cooper Foster Park Road for the construction of the Martin's Run Wetland Complex, to be paid from the Storm Fund (4030), as approved by Lorain City Council on May 18, 2020, through Ordinance 041-20.

A REQUEST FROM THE DEPARTMENT OF PUBLIC PROPERTIES TO AUTHORIZE THE SAFETY/SERVICE DIRECTOR TO ENTER INTO AN AGREEMENT WITH DOVIN FUNERAL  AND CREMATION SPECIALISTS, FOR THE INDIGENT BURIALS FOR THE CITY OF LORAIN FOR A PERIOD OF TWO (2) YEARS WITH AN OPTIONAL ONE (1) YEAR EXTENSION AT A RATE OF $525.00 PER CREMATION, IN AN AMOUNT NOT TO EXCEED $15,000.00  PER YEAR.

d.   The Fire Department requests approval to subscribe to Emergency Reporting (Backdraft OpCo LLC) to replace our FireHouse software which has changed ownership and will no longer be supported after September, 2021.  The first year cost is $20,000, which the second year cost estimated to be $6,000.  Supporting documents attached.

e.   A request from the Engineering Department to award to the lowest and best bidder, Perk Company, Inc., the 2615 E. Erie Ave Storm Outfall Improvement Project with a base bid of $124,521.00 and a 10% contingency for the certified contract amount of $136,973.10. One other bid in the amount of $211,760.00 was received by Schirmer Construction, LLC. This work will be funded through 4030.C403.6300.1500 and was authorized by Ordinance 51-21.

f.   The Small Business Stabilization Program assists businesses impacted by the Coronavirus Pandemic. Lorain Family Dental located at 4560 Oberlin Ave, Lorain, OH 44053 applied to receive a Forgivable Loan in the amount of $6,728.18. This is an amendment to the original approved BOC approval date of February 17, 2021 and amended request approved on BOC approval date of July 7, 2021 due to an account change.
