Care Coordination and Family Caregiver Report
Care Coordination:
Care Coordination staff is focused on case management duties as they complete assessments and process assistance for non-assessment services. Staff continues to work to meet State and internal goals while tracking expenditures. Case file review at the end of month five of FY18 indicates AAA staff may double the number of clients served this fiscal year at the current rate of requests. The AAA is experiencing, non-assessment clients (ERS, Income Support and Health Maintenance) requests coming in at higher request than usual for this time of the year. We have seen an impact to individuals who need assistance with Income Support as utility bills and propane continue to rise. The AAA is working on internal change procedures to accommodate the demand of calls for service while meeting requirements to our funders HHSC.

Family Caregiver:
The AAA rural area has ten donated space heaters left for both Access and Caregiver families. Each year, AACOG staff, donate school supplies at the start of a new fiscal year. AACOG staff understand the need for school supplies and their commitment and passion to donating has brought joy and relief to Grandparents raising grandchildren in rural counties.
